BE A MODEL

We are always looking for models to undergo treatments by our students on their Mae Cosmetics Academy practical course days. Your treatment will be carried out by a student in training at our academy under the supervision of our technicians. We offer a range of various treatments at a reduced price (see model release form for details). Please expect your treatments to take slightly longer than they would normally. If you'd like to be considered for any upcoming course dates, please fill out the form below. It is vital that you answer the questions honestly to ensure we can carry out the treatment safely and properly, failure to do so could result in health complications, compromised results or your treatment being refused. Ensure you have read the Model Release Form in full before submitting your registration.

Course dates, frequency and duration will vary and so we will contact prospective models when preparing for upcoming dates to arrange appointments and carry out a full consent process. You will only be considered for the treatment(s) you select below, providing you are a suitable model candidate following technician consultation.

MEDICAL QUESTIONNAIRE + MODEL RELEASE DECLARATION

Please fill this form out truthfully and to the best of your knowledge. A full up-to-date medical history questionnaire and consent process with thorough consultation and treatment plan will be carried out prior to your treatment if you are selected as a model.
